Rebate claim should be submitted with a dated sales receipt, dated eye exam receipt, and original end panels from the contact lens boxes.

Please note:  Rebate claims must be submitted within 60 days of lens purchase. All receipts must be from the same eye care practitioner who prescribe your c

ontacts, or from a location affiliated with that practitioner.
